Hazardous event is the combination of a hazard (caused by malfunctioning behaviour of an item with respect to its design intent) and a specific operational situation.
Explanation
A hazard is a process, phenomenon or human activity that may cause loss of life, injury or other health impacts, property damage, social and economic disruption, or environmental degradation. For example, “a tree falling on a street”.
